93 pages, 1996, Nordic nuclear safety research programme. Ølgaard compiles the known accidents involving nuclear-powered ships — overwhelmingly submarines, overwhelmingly Soviet — including reactor casualties, losses at sea, and the radiological consequences of each.

The value is aggregation. Individual incidents are separately documented and often separately classified; what almost never exists is one document that puts the full set side by side with consistent technical description. That is what makes this an answer to “rarest PDF” rather than merely “obscure PDF.”

Useful as a base-rate document: if you want to reason about how often a naval reactor actually fails, and how badly, this is the population rather than the anecdote.
